There are two different forms for team officials to use to register -- the difference is the age of the team official, not the age of the players on the team!

This form to be completed by the applicant (coach/assistant coach/manager) if the applicant is 18 years of age or older
This form to be completed by a parent or legal guardian if the applicant (coach/assistant/manager) is under 18 years of age
Coaching Courses:
Making Headway (all EOSL)
*Respect in Sport (all coaches/assistant coaches U8+)
Make Ethical Decisions (all age groups, EOSL)
Learn to Train (U9-U12, EOSL)
Soccer for Life (U13+, EOSL)
*Coaches that have completed Respect in Sport for another sport (ie hockey) do not need to take the course again for soccer, however, this course expires after 5 years and must be taken again to maintain coach qualifications.
Police Records Check for working with the Vulnerable Sector: required for all team officials for youth teams playing in the EOSL (competitive teams), valid for 3 years from date of completion by Police. A letter of volunteer status can be provided for registered team officials.
